To be eligible to use our services you must meet the following guideline:

Want quality health and dental care!

We accept Medicaid, Medicare, and private insurance plus we also offer a sliding fee scale for those without insurance.  We accept patients of all ages.

Rates

Services at Open Door are based on a sliding fee scale, which means that people who are uninsured can get discounts based on their income and household size. 

Currently, if you have a household of four, you qualify for a discount if your total household income is less than $3,534 per month.

For medical services you can get up to an 80% discount.

For dental services you can get up to a 75% discount.

If you are already on our sliding fee scale, please bring at least $15 each visit to put towards that day's charges.  If you are not on the sliding fee scale and do not have one month of household income information available to bring with you, you will need to bring $50.

Payment Arrangements

We understand that sometimes you can't pay your entire bill at the time of service.  Please make sure that you bring at least $15 dollars to each appointment.  A few weeks after your appointment you will receive a bill for the rest of the cost of your visit.  If you are on the sliding fee scale, this bill will already be reduced by the appropriate amount.  If you are unable to pay the full amount of the bill, you can make payment arrangements.  As long as you are making consistent payments toward your bill, you will not be terminated from care due to non-payment.
